I have read and agree to the following:

• I agree to play 100 holes of golf for the cause
  
  
• I agree to have fun and enjoy a great day of golf
  
  
• I agree to set an example with a contribution of my own
  
  
• I agree to only have one ball in play the entire 100 holes
  
  
• I agree to play my golf during the dates identified for this event
  
  
• I agree to register on our website and set up my own personal fundraising page
  
  
• I agree to upload a .jpg picture of myself for use on this website
  
  
• I agree to email everyone in my address book with a friendly message letting them know what I am doing and how they can support the cause, including a link to my personal web page
  
  
• I agree to thank all my supporters within one week of the event
  
  
• I will strive to raise a minimum of $20 per hole ($2,000) in contributions
  
  
• I agree to recruit at least one new person to participate next year
  
  
• I realize that I am representing the PGA, the Bayou District Foundation, as well as associated charities, programs and all my sponsors while engaged in this fundraiser.  I will conduct myself in accordance with the Bayou District Foundation Code of Ethics for the entirety of the event.  In the event that I cannot fulfill my obligations of 100 holes, I will inform the Bayou District Foundation immediately so that accommodations can be made.
